Prescription pad or letterhead
Choose what to make at the top. A prescription pad is a page for writing on by hand: the doctor's details at the top, lines for the patient, the Rx sign and room to write, the clinic's address and timings at the foot. A letterhead is a business's, office's, school's or association's page for letters, quotations and notices. Both share the name, logo, address and colours, so switching between them keeps what you have typed.
The details
- Doctor (pad only) — the name, qualifications, specialty or post, and registration number. There is room for up to two doctors. A registration number is printed after "Reg. No." unless you typed that yourself.
- Clinic, business, office or school — the name and a line under it, such as
Family medicine & diabetes careorLicensed electrical contractors since 1998. - Logo — a PNG, JPG, WebP or SVG picture. It is made smaller (520 pixels on its longer side at most) and kept in the browser with the rest. A PNG with a see-through background looks best.
- Address in up to three lines, the phone, email and website, and another line for a clinic registration, GSTIN or CIN.
- Timings (pad only) in up to two lines, and a line at the foot of the page, such as "Not valid for medico-legal purposes" or a registered office address.
Every field can be typed in English or in an Indian script; the page fetches a matching font the first time it is needed so the names print correctly. Long names are made a little smaller and then put on two lines, and when the phone, email and website do not fit on one line they are set one under the other.
Where things go
On a pad with one doctor, the doctor's name and details are on the left and the clinic's name and logo on the right. With two doctors, the clinic goes across the top and one doctor sits on each side under it. On a letterhead, the logo and name are on the left and the address and contact details on the right. The Centred style puts everything in the middle instead.
The look
- Classic — a double rule under the header, the details in the page's colour.
- Centred — the logo, name and details centred, with a fine double rule.
- Colour band — the header on a band of colour in white lettering, with a matching band at the foot. A logo sits on a small white card so it still shows on the colour.
- Side strip — a narrow band of colour down the left edge of the page.
Pick one of six colours or any colour of your own, serif or plain lettering for the names, and A4, A5 or Letter paper.
What is on the page
A pad always has lines for the patient's name, age, sex and date. You choose whether it also has a row for weight, BP, pulse, temperature and SpO₂, a column down the left for complaints, findings, diagnosis and investigations, a line for the next visit, and a signature line — with one doctor, their name is printed under it. A letterhead can have lines for the reference number and the date under the header, and the logo printed faintly in the middle of the page.
The preview draws the page as you type. Fields you have not filled in show grey example words so you can see where they go; the files leave them out.
The files
- Download PDF — the page as a 300 dpi picture, sharp on a home or office printer. Print as many as you need, or take it to a local press. A press doing offset printing with exact ink colours may prefer to set it up again from the PDF.
- Two A5 on an A4 sheet (with A5 paper) — two A5 pads side by side on an A4 sheet laid sideways, to print on an ordinary printer and cut in half.
- Print — straight to the printer at the paper size you chose.
- Word file to type on — an empty Word document with the letterhead behind the page and the margins set so your typing starts under the header and stops above the footer. The letterhead repeats on every page and cannot be moved by accident while you type. The patient lines and the Rx sign are left out, so the whole middle of the page is for typing. It is made for Microsoft Word; other programs may place the picture differently.
A pad cannot be made without the doctor's name, nor a letterhead without the name of the business or office — the page asks for it first.
Keeping it
Everything you type, and the logo, is saved in this browser on this device as you go, so the design is there next time. Nothing is uploaded: the preview, the PDF and the Word file are all made on your device. Start again clears it all.
Two papers most clinics and offices need. A prescription pad carries the doctor's name, qualifications and registration number, the clinic's address and timings, and lines for the patient's details, ready to write on by hand. A letterhead carries a business's, office's, school's or association's name, logo and address for letters, quotations and notices. Both are drawn as you type and saved in this browser for next time.
What does the prescription pad show?
The doctor's name, qualifications, specialty and registration number, the clinic's name and logo, the address, phone and timings, lines for the patient's name, age, sex and date, a row for weight, BP, pulse, temperature and SpO₂ if you want it, the Rx sign, a line for the next visit and a signature line.
Can I type letters on the letterhead in Word?
Yes. The Word file has the letterhead behind the page and the margins set so your typing starts under the header and stops above the footer. It repeats on every page. It is made for Microsoft Word.
Can I print A5 prescription pads?
Yes. Choose A5 for an A5 page, or download two A5 pads side by side on one A4 sheet to print on an ordinary printer and cut in half.
Is the PDF good enough for a printing press?
The PDF is the page as a 300 dpi picture. That prints sharp on a home or office printer and is usually fine for a local press. For offset printing with exact ink colours, a press may prefer to set it up again from your PDF.
Is my information uploaded?
No. What you type and your logo stay in this browser on this device. The PDF and the Word file are made on your device.
